Program Description
"The federal government has announced a major national initiative to improve care transitions. The Center aims to help thousands of communities nationwide as they respond to this initiative and move to create seamless, comprehensive care."

The "Care Transition" Programs' main focus is to support and advocate for improving patient care as patients move among care settings.
The “Center for Elder Care and Advanced Illness Care Transitions” Program provides help for those transitioning through care with the “Care Transitions Guide.” The guide contatins information to help patients, elderly, caregivers and families “fix” care transitions, understand the hospital discharge process and how to avoid hospitalization readmission. Other topics include coalition building, setting priorities and palliative care programs and services.
The “Center for Elder Care and Advanced Illness” provides resources to help make care transitions seamless and safe through www.medicaring.org a website featuring a Care Transitions Search Widget, a blog and an introductory guide for those ready to lead care transitions reform in their own communities.
The “Center for Elder Care and Advanced Illness” also focuses on caregiving. Their aim is to improving social support for family caregivers who can be an integral to creating health care reform.
The “Altarum Institute” is a research and consulting organization that uses independent research and client-centered consulting to create systems to improve the health of all individuals.
