A Dementia care manager works with primary care doctors and caregiver families to coordinate a person-centered personalized care plan.
The “UCLA Alzheimer’s and Dementia Care” Program services include:
-In-person visit with a Dementia care manager
-Personalized dementia care plan developed with the referring physician
-Follow-up phone calls and in-person visits to make sure the dementia care plan is implemented or modified as needed
-24/7, 365 day a year access for dementia caregivers assistance and advice.
In-person visits are covered by Medicare. Telephone calls and many other services such as, support groups, educational programs, and referrals to community organizations are offered for free.
The “UCLA Alzheimer’s and Dementia Care” Program offers “Beyond Alzheimer's,” a free support group in Los Angeles for caregivers, friends and loved ones of someone with Alzheimer's disease or dementia. The “Alzheimer's Caregiver Education Lecture Series” is also available for support.
GeroNet is a division of UCLA’s educational, research and clinical resources on aging and the health care of older persons and is accessible to the public, patients and their advocates, students, researchers and health care providers.
