Program Description

“First Link is a referral program that helps healthcare providers connect a person living with dementia to their local Alzheimer Society, from the time of diagnosis and throughout the duration of their experience living with the disease. That way, people living with dementia and their families receive education and support in their community as early as possible.”

The Alzheimer Societies of Canada First Link Program works with physicians, geriatric specialists, nurses, social workers, occupational therapists, pharmacists, social service workers and others to help support those with Alzheimer’s.
The program aims to help those newly diagnosed with Alzheimer’s to receive assistance quickly and begin taking steps to plan for the future.
The Alzheimer Societies of Canada First Link Program provides access to:
- One-on-one or group support
- Informative workshops
- Local health and community services
- Meet ups with other people with Alzheimer’s and exchanging experiences
- Practical tips and strategies
- Assistance in planning for the future by helping develop competency and resilience during the Alzheimer’s journey.
