Program Description
“Qualified tribes can provide community-based LTSS programs and, in most cases, provide them in ways that reflect their unique needs, culture, values, and community expectations. Most AI/AN families want to care for their loved ones in ways that preserve their dignity and honor cultural traditions.”
“To provide direct care, a tribe must be accredited to operate as a home care agency or facility-based LTSS program. Once the program is accredited, a tribe can directly provide LTSS to its members.”

Providing access to long-term care services aims to reduce the health care disparities which exist for Native communities.
American Indian and Alaska Natives experience high rates of some chronic illnesses such as diabetes, cancer, and heart disease and may require long term supportive care services.
Long-Term Services and Supports can be either home/community-based care or facility-based care each offering differences in how care is delivered depending on where the elderly person lives.
Tribes can offer elder services which range from providing referrals, assistance with benefits, partnering with long-term care agencies all the way to operating as a care facility providing direct elder care services themselves.
Long-Term Services and Supports program for tribes may include how to set up the following assistance:
- Tribal Nursing Homes
- Dementia and Long-Term Care
- Hospice and Palliative care
- Caregiver support
- LTSS for Native Veterans
- Elder abuse prevention
- Traditional Foods in Long-Term Care
