"Community Choice helps nursing facility residents and hospital patients explore various community-based alternatives by providing information about in-home services, housing alternatives, and community programs. The program encourages participants to make well-informed decisions about what is best for their long-term care."

The program provides helps families decide about long-term care.
A senior resident's discharge from a nursing care home facility is coordinated with other state and community agencies and necessary support services are provided to help make the transition to senior housing.
The "Community Choice" Program Counselors work with seniors, caregivers and families to:
-Provide information on special senior housing, community programs and in-home services
-Explain any financial or medical requirements
-Coordinate with the nursing care facility discharge planner to find services which support senior's dignity, choice and independence
-Help make arrangements so that seniors are housed in a happy, safe and appropriate community setting
