The “Community Choice” Program helps nursing facility residents and hospital patients learn how to find the right nursing home or how to choose nursing home alternatives.
Senior housing assistance options may include in-home care services, housing alternatives, such as senior co-housing, home sharing and other programs.
Seniors can learn how to choose the best long-term care with the help of “Community Choice” Program counselors, registered nurses and social workers. Advisors meet with senior nursing home residents to assess their individual health care needs, to determine the appropriate level of care required and to suggest appropriate alternatives. They are also able to help with financial and medical eligibility requirements.
