The "Patient Education and Caring: End-of-Life (PEACE) Series" Program was developed by a group of physicians and patient advocates.
Brochures are available in print and online and advance health care and end of life topics include:

-Making Medical Decisions for a Loved One at the End of Life
An additional resource by the "American College of Physicians" entitled "Home Care Guide for Advanced Cancer," covers end of life and hospice training such as:
